Rachael is a Registered NMC Midwife who has practiced in both hospital and community settings; she is in current practice as a private midwife looking after all aspects of maternity care within community and home settings.

She has a clinical background includes labour wards, high-risk obstetric care, antenatal and postnatal ward roles, specialist midwifery roles, specialist infant feeding, high risk pregnancy care, maternity risk management, obstetric emergency

Rachael has had a Senior Manager role within an Integrated Care Board; having a leading role on service improvement, transformation and service commissioning. She has a comprehensive understanding of midwifery standards of practice and has experience in implementation and ensuring adherence to these standards.

Rachael is experienced in reviewing clinical records and writing detailed incident reports, forming robust opinions on standards of practice; implementing learning outcomes to raise standards within clinical settings.

Rachael accepts instructions to undertake liability reports in relation to:

• Proper escalation of issues and concerns.
• Recording of midwifery notes and documentation.
• Hospital including CTG interpretation and management.
• Home birth settings including appropriate management, timely and appropriate escalation of concerns.
• Delays in response through antenatal to postnatal care.
• Identifying risk factors of early complications after birth, for both the mother and baby.
• Identification of acute psychosis.
• Obstetric emergency, including administration of care around hypoxic brain injury, shoulder dystocia injuries, hemorrhages and eclampsia.
