CASE MANAGEMENT
Fiona Johnson Ltd has experienced independent case managers who specialise in the rehabilitation of people who have had serious, complex and catastrophic injuries.
We follow the Rehabilitation Code (2015) and act on a joint instruction basis, we provide nationwide Case Management services for moderate to complex and catastrophic injuries. Our services include telephone and field based initial needs assessments, combined assessments, clinical, rehabilitation and injury case management services.

ABOUT OUR SERVICE
What our Case Managers can do for you:
Complete Immediate Needs Assessments based on the examination of an individual and review of their current status, daily living activities and rehabilitation needs under the Rehabilitation Code 2015 including costs for care, rehabilitation and case management;
Review any applicable records, reports and other relevant documentation including the client’s instruction letter;
Maintain records/notes of all actions;
Visit and assess the Claimant in their home environment;
Provide bimonthly, or more frequent, progress reports on the rehabilitation status / progress or otherwise against the goals set;
Attend a Court, Tribunal or Arbitration Hearing, as directed, to give evidence under Oath in matters relating to the instructed case.
Attend case conferences as directed by the client, and as the need arises.
